6K a year 17 hours ago 17 hours ago 17 hours ago Case Manager JOB SUMMARY The Non-Clinical Case Manager provides non-clinical student case management, including resource identification, referrals and follow up to resources, and basic-needs support for students who have challenges with their health/wellness, academics, housing, finances, academics, basic needs resources, public benefit assistance, behavior, safety and other aspects of their general welfare.
The University reserves the right to change or assign other duties to this position.
DESCRIPTION OF DUTIES AND TASKS:
Manage a revolving student case load.
Provide individualized care and case management services so students can meet their goals.
This includes intake, needs assessment, resource management, service planning, and monitoring and evaluation.
Coordinate and monitor referrals to support services for students, including referral and liaison services with housing agencies, social services, mental and physical health services, financial support agencies, food services, legal services, etc.
, through direct communication with students or consultations with faculty, staff, or parents.
Implement systems for follow-up with students to ensure continuity of care, access of resources/referrals, and appropriate intake for campus support services.
Offer consultation to students, faculty, staff, parents, other healthcare providers, for purposes of referring students to community and private resources (food banks, shelters, etc.
) as appropriate.
Maintain full, accurate, and up-to-date case records and prepare necessary statistical reports.
Adhere to the standards of confidentiality.
Work some evenings and weekend hours for campus outreach events or emergency/campus crisis/disaster situations, as needed.
Provide non-clinical post-hospitalization support to students returning from a mental/behavioral health setting and transitioning back to the University environment.
Assist students in navigating a variety of academic and financial appeals.
Assist students with public benefits applications for SNAP/Medicaid, etc.
Consult regularly with relevant departments on campus regarding services and care to maintain strong collaborative relationships with campus partners, including Academic Success, Residence Life, Health, Counseling, Student Life.
Establish working relationships/partnerships with community agencies, services and resources to meet students' basic needs, and serve as a liaison to community and private mental health providers, social services, and hospitals.
Other duties as assigned.
JOB ACCOUNTABILITIES -Case management of students in need of additional assistance and documentation of cases.

EDUCATION/SPECIALIZED KNOWLEDGE/PROFESSIONAL CERTIFICATIONS Required:
Bachelor's degree in social work, psychology, criminal justice, or another related human services field.
Preferred:
Experience in a higher education setting.
Prior case management experience WORKING CONDITIONS Grant-funded position for one year for 20 hours/week, 30 weeks/year during the fall and spring semesters.

The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
